Transportation Services Agreement

This Transportation Services Agreement ("Agreement") is entered into between Nurse Next Door Mississauga ("Provider") and the client requesting transportation services ("Client").

1. Services Provided

The Provider agrees to arrange non-emergency transportation services for the Client to and from medical appointments, personal errands, and other destinations as agreed upon at the time of booking.

2. Pricing

All transportation services are billed at a rate of $20.00 per 15 minutes (charged in 15-minute increments), applicable to each of the service types offered:

• Curb to Curb — $20.00 per 15 minutes
• Door to Door (Drop-off Only) — $20.00 per 15 minutes
• Door to Door (Wait & Return) — $20.00 per 15 minutes
• Caregiver to Client — $20.00 per 15 minutes
• Customized Trip — $20.00 per 15 minutes

Billable time is measured from the scheduled pickup to the completion of the trip. The Client will be advised of estimated costs at the time of booking.

3. Client Responsibilities

• Provide accurate pickup and drop-off addresses at the time of booking.
• Be ready at the agreed pickup time. A wait time of up to 10 minutes will be observed before the ride is considered a no-show.
• Notify Nurse Next Door Mississauga at least 24 hours in advance if a ride needs to be cancelled or rescheduled.
• Treat all drivers and staff with respect.

4. Cancellation Policy

Cancellations made less than 24 hours before the scheduled ride may be subject to a cancellation fee. Repeated no-shows may result in suspension of transportation services.

5. Liability

Nurse Next Door Mississauga will take reasonable care to ensure safe and timely transportation. The Provider is not liable for delays caused by traffic, weather, or circumstances outside of its control.

6. Privacy

Personal information collected during booking is used solely for the purpose of arranging transportation services and is handled in accordance with applicable privacy legislation.

7. Agreement

By signing below, the Client acknowledges that they have read, understood, and agree to the terms of this Transportation Services Agreement.
