Transportation Services Agreement

This Transportation Services Agreement ("Agreement") is entered into between Nurse Next Door Mississauga ("Provider") and the client requesting transportation services ("Client").

1. Services Provided

The Provider agrees to arrange non-emergency transportation services for the Client to and from medical appointments, personal errands, and other destinations as agreed upon at the time of booking.

2. Pricing

All transportation services are billed at a rate of $20.00 per 15 minutes (charged in 15-minute increments), applicable to each of the service types offered:

• Curb to Curb — $20.00 per 15 minutes
• Door to Door (Drop-off Only) — $20.00 per 15 minutes
• Door to Door (Wait & Return) — $20.00 per 15 minutes
• Caregiver to Client — $20.00 per 15 minutes
• Customized Trip — $20.00 per 15 minutes

Billable time is calculated from the scheduled pickup time until the transportation provider has completed the service and departs from the Client’s final destination. The Client will be advised of the estimated cost at the time of booking. If the transportation service extends beyond the scheduled end time, the Client understands and agrees that billing will be based on the actual time required to complete the service, calculated at the applicable hourly rate.

Recurring Transportation Appointments: If you require transportation on a recurring basis such as weekly, bi-weekly, monthly, or multiple fixed appointments each month, our team will work with you to schedule these in advance and ensure consistency. These appointments may be billed at a flat monthly rate, based on frequency and distance, and will be reflected in your personalized Transportation Plan. All recurring services are subject to availability and scheduling coordination.

Notes Regarding Rates:

• Additional services are subject to rates which are agreed upon prior to service.
• Any last-minute changes and/or urgent requests may be subject to additional rates.
• Parking fees, tolls, or incidental costs may be added to your invoice.
• All rates are subject to HST.

Statutory Holidays

The following days are considered holidays and are billed at a rate of time and a half: New Year’s Day, Family Day, Good Friday, Victoria Day, Canada Day, Civic Holiday (Ont.), Labour Day, Thanksgiving Day and Boxing Day. Service on December 25th (Christmas Day) will be billed at double time.

3. Client Responsibilities

• Provide accurate pickup and drop-off addresses at the time of booking.
• Be ready at the agreed pickup time. A wait time of up to 10 minutes will be observed before the ride is considered a no-show.
• Notify Nurse Next Door Mississauga at least 24 hours in advance if a ride needs to be cancelled or rescheduled.
• Treat all drivers and staff with respect.

4. Cancellation Policy

Cancellations made less than 24 hours before the scheduled ride may be subject to a cancellation fee. Repeated no-shows may result in suspension of transportation services.

5. Billing and Payment Policy

Invoices are issued on the 16th and last day of each month. Interest of 2.5% per month may be charged on accounts with unpaid balances over 30 days. We have the absolute right, without limitation or penalty, to stop all work in the event of billing disputes and/or where delinquent charges are due to us.

Credit card transactions: A 3.5% transaction fee will be applied to all credit card transactions. This fee covers the costs associated with processing credit card payments.

You agree to reimburse Nurse Next Door for any out-of-pocket expenses directly related to such services including, but not limited to, parking fees and tolls.

6. Liability

Nurse Next Door Mississauga will take reasonable care to ensure safe and timely transportation. The Provider is not liable for delays caused by traffic, weather, or circumstances outside of its control.

7. Privacy

Personal information collected during booking is used solely for the purpose of arranging transportation services and is handled in accordance with applicable privacy legislation.

8. Agreement

By signing below, the Client acknowledges that they have read, understood, and agree to the terms of this Transportation Services Agreement.
